    TL;DR

    Raising HDL with drugs has repeatedly failed to reduce cardiac events, so HDL is a marker rather than a target. Exercise, weight loss and stopping smoking raise it in the way that actually correlates with benefit.

    Why It Matters

    Low HDL is associated with higher cardiovascular risk, but Mendelian randomisation and CETP inhibitor trials show raising it pharmacologically does not lower events. HDL function matters more than concentration.
