TL;DR
Elevated fibrinogen is an independent cardiovascular risk marker, but it is mostly a downstream signal of inflammation, smoking and inactivity. Treat the cause; nattokinase and omega-3 have only modest supporting data.
Why It Matters
Fibrinogen above roughly 4 g/L is associated with increased risk of myocardial infarction and stroke, reflecting both clotting tendency and systemic inflammation. It rises with smoking, obesity, infection and sedentary behaviour.

Optimization Protocol
Stop smoking, train aerobically at least 150 minutes weekly, and reduce visceral adiposity — these have the largest documented effects. Add omega-3 at 2 g EPA/DHA daily. Nattokinase 2000 FU may be trialled but should never replace prescribed anticoagulation. Recheck fibrinogen at 12 weeks.
