Overview
Memory complaints span a wide spectrum. Normal age-related change involves slower retrieval with intact recognition and preserved function. Mild cognitive impairment involves objective deficits on testing with daily function largely maintained, and roughly 10 to 15 percent of those affected progress to dementia annually. Dementia involves cognitive decline that impairs independent living. The single most useful clinical step is excluding reversible contributors, because a meaningful minority of memory complaints have a treatable cause. Vitamin B12 deficiency, hypothyroidism, depression presenting as pseudodementia, obstructive sleep apnoea, chronic alcohol use, and anticholinergic medication burden all produce genuine cognitive impairment that improves when addressed. Anticholinergic load in particular is under-recognised and is associated with dementia risk in large cohort studies. For risk reduction, the 2024 Lancet Commission identified 14 modifiable factors accounting for around 45 percent of dementia cases worldwide, including hearing loss, hypertension, physical inactivity, smoking, diabetes, social isolation, depression and untreated visual impairment. Hearing aid use in high-risk adults reduced cognitive decline in the ACHIEVE trial, one of the few positive randomised results in this field. The multidomain FINGER trial combining diet, exercise, cognitive training and vascular monitoring also showed benefit. The supplement picture is largely disappointing. Ginkgo biloba failed to prevent dementia in the large GEM trial. Multivitamins showed modest cognitive benefit in COSMOS-Mind, an intriguing but preliminary signal. Citicoline and phosphatidylserine have limited support, omega-3 helps mainly where intake is low, and B vitamins slow atrophy only in people with elevated homocysteine.

Root Causes
Memory loss reflects either reversible physiological disruption — B12 or thyroid deficiency, depression, sleep deprivation and apnoea, anticholinergic and sedative medication, alcohol, delirium — or progressive neuropathology, most often Alzheimer amyloid and tau pathology, vascular cognitive impairment from small vessel disease, Lewy body disease, or frontotemporal degeneration. Vascular risk factors accelerate almost all of these pathways.

When to See a Doctor
Book an assessment if memory problems are noticed by family, interfere with work or finances, have progressed noticeably over six to twelve months, or are accompanied by personality change, disorientation or word-finding difficulty. Seek urgent care for sudden confusion, which suggests delirium, stroke or infection rather than dementia. Review all medications for anticholinergic burden before assuming decline is irreversible.
Supplements Studied For This
Vitamin B12
B12 matters for memory exactly when it is missing — deficiency causes real, partly reversible cognitive impairment, but supplementing normal levels has no demonstrated memory benefit.
Citicoline (CDP-Choline)
Reasonable evidence for age-associated memory lapses in adults over 50; weaker for young healthy memory.
Ginkgo Biloba
Not recommended for age-related forgetfulness. The best-quality evidence is negative, and it interacts with blood thinners.
Phosphatidylserine
Phosphatidylserine may modestly help everyday memory lapses in older adults, though effect sizes are small and formulation-dependent.

Suggested Pattern
The MIND diet, a Mediterranean-DASH hybrid emphasising leafy greens, berries, nuts, olive oil and fish while limiting red meat, fried food, butter and pastry, is associated with slower cognitive decline in cohort studies. The randomised MIND trial found improvement in both arms with no significant difference over three years, tempering the observational enthusiasm. Overall, patterns that protect blood vessels appear to protect cognition, which is the more robust and consistent conclusion.

- Berries, particularly blueberries and strawberries

- Nuts, especially walnuts
- Olive oil as the primary fat
- Legumes and wholegrains
- Coffee and tea in moderation
Avoid
- Fried and fast food
- Pastries, sweets and refined baked goods
- Processed and red meat in quantity
- Butter and stick margarine in large amounts
- Excess alcohol, which is directly neurotoxic
- Ultra-processed foods associated with faster decline in cohorts
