    TL;DR

    A sensation of faintness or near-blackout, most often from a transient drop in cerebral perfusion. Orthostatic hypotension, dehydration, anaemia and medication effects account for the majority. Hydration and salt help true orthostatic cases; iron corrects it where anaemia is the cause. Cardiac and neurological causes must be excluded.

    Overview

    Lightheadedness describes the feeling of being about to faint — a woozy, drifting, greying-out sensation. It is worth separating from true vertigo, which is a spinning illusion of movement usually arising from the vestibular system, because the causes and workup differ substantially. Most lightheadedness is presyncopal: a brief fall in cerebral perfusion pressure. Orthostatic hypotension, defined as a drop of 20 mmHg systolic or 10 mmHg diastolic within three minutes of standing, is the most common identifiable mechanism, and its prevalence rises steeply with age and with the number of antihypertensive, diuretic, alpha blocker and antidepressant medications taken. Volume depletion from inadequate fluid intake, heat, vomiting or diarrhoea compounds it. Non-circulatory causes matter too. Iron deficiency anaemia reduces oxygen-carrying capacity and commonly presents as lightheadedness with fatigue and breathlessness. Hypoglycaemia, anxiety with hyperventilation, vestibular migraine and postural orthostatic tachycardia syndrome each have distinctive patterns. POTS in particular affects predominantly young women and is defined by a sustained heart rate rise of 30 bpm or more on standing without hypotension. The clinical priority is excluding dangerous causes: arrhythmia, structural heart disease, gastrointestinal bleeding and cerebrovascular events. Lightheadedness that occurs during exertion, while supine, or with palpitations or chest pain warrants cardiac assessment rather than a supplement.

    Root Causes

    The final common pathway is reduced cerebral perfusion or oxygen delivery. This arises from orthostatic hypotension, volume depletion, autonomic dysfunction including POTS and diabetic neuropathy, medication effects, cardiac arrhythmia or outflow obstruction, anaemia from iron deficiency or occult bleeding, hypoglycaemia, and hyperventilation-driven cerebral vasoconstriction in anxiety.

    Diagnostic Markers

    • Lying and standing blood pressure at 1 and 3 minutes
    • Heart rate response to standing — a rise of 30 bpm or more suggests POTS
    • Full blood count and ferritin for anaemia and iron deficiency
    • Capillary or fasting glucose
    • Electrolytes, urea and creatinine for volume status
    • ECG, with Holter monitoring if arrhythmia is suspected
    • B12 and folate
    • Echocardiogram where a murmur or exertional symptoms are present
    • Tilt table testing in unexplained recurrent presyncope

    When to See a Doctor

    Seek emergency care for lightheadedness with chest pain, palpitations, breathlessness, slurred speech, weakness on one side, black stools or actual loss of consciousness. Book prompt review for symptoms during exertion or while lying flat, for recurrent near-fainting, or if you take blood pressure or heart medication. Do not start salt loading without medical advice if you have hypertension, heart failure or kidney disease.

    For genuine orthostatic intolerance, the dietary approach is counterintuitive relative to general health advice: increased fluid to 2 to 3 litres daily and increased sodium to roughly 6 to 10 g of salt improves plasma volume and standing tolerance, provided blood pressure and cardiac and renal function permit it. Large high-carbohydrate meals worsen symptoms by diverting splanchnic blood flow, so smaller frequent meals help. Where anaemia is the cause, iron intake with vitamin C and avoidance of tea with meals is the priority.

    • Iron-rich foods: red meat, liver, lentils and fortified cereals

    • Potassium-containing foods for general cardiovascular balance

    Avoid

    • Alcohol, which is both a vasodilator and a diuretic
    • Large heavy meals that trigger postprandial hypotension
    • Prolonged fasting and skipped meals
    • Tea and coffee with iron-rich meals — tannins block absorption
    • Very hot baths and saunas that worsen vasodilation

    Dizziness and lightheadedness account for roughly 4 percent of emergency department visits, and orthostatic hypotension affects around 6 percent of the general adult population and up to 20 percent of those over 65.
