Overview
Gallbladder disease usually means gallstones: hardened deposits, mostly cholesterol, that form when bile becomes supersaturated. Most stones are silent and never need treatment. Problems begin when a stone obstructs the cystic duct, producing biliary colic — a steady, severe pain in the right upper abdomen, often after a fatty meal, lasting 30 minutes to several hours. If obstruction persists, acute cholecystitis follows, with continuous pain, fever and a need for urgent care. Stones that migrate into the bile duct cause jaundice, cholangitis and pancreatitis, which are emergencies. Risk factors cluster around cholesterol saturation of bile: female sex, age, obesity, rapid weight loss, pregnancy, oestrogen therapy and family history. Notably, very low-fat diets and crash dieting promote stones by leaving bile stagnant in the gallbladder, which is why steady, moderate fat intake is protective even during weight loss. Ultrasound confirms stones reliably. Symptomatic gallstones are treated by surgical removal of the gallbladder, typically laparoscopically; the body adapts well to its absence, digesting fat normally with minor adjustments.

Root Causes
Gallstones form when bile contains more cholesterol than bile acids and lecithin can hold in solution, so cholesterol crystallises. Gallbladder stasis — from fasting, very low-fat diets or pregnancy hormones — lets crystals grow into stones. Haemolysis and liver disease produce pigment stones through excess bilirubin. Obesity and insulin resistance increase hepatic cholesterol secretion into bile, which is why metabolic health and gallstone risk track together.

When to See a Doctor
Seek same-day care for continuous right upper abdominal pain lasting more than a few hours, especially with fever. Call emergency services or attend immediately for jaundice, fever with shaking chills, severe constant pain, or pain with vomiting that does not settle — these suggest cholecystitis, cholangitis or pancreatitis. Recurrent colicky attacks after meals warrant elective review, since symptomatic stones tend to recur and complications accumulate.

Diet influences both stone formation and symptom provocation. Regular meals with moderate fat keep the gallbladder contracting and bile flowing; prolonged fasting and very low-fat diets are paradoxically stone-promoting. Rapid weight loss mobilises cholesterol into bile and is a leading trigger — weight loss should be gradual, no more than about 0.5-1 kg weekly. During symptomatic periods, lower-fat meals reduce colic provocation. After gallbladder removal, most people tolerate a normal diet, though some do better introducing fats gradually and reducing very fatty meals.
