- Evidence is insufficient. The rationale comes from cell and animal work: vitamin D receptors are expressed in adipocytes, calcitriol influences adipogenesis and leptin secretion in vitro, and observational studies find low 25(OH)D alongside high leptin in obesity. Human trials have not demonstrated changes in leptin sensitivity, and leptin sensitivity is not directly measurable in people - it is inferred from circulating leptin relative to fat mass, a crude proxy. Vitamin D supplementation trials in obesity have consistently failed to produce meaningful weight or metabolic change.
