    Overview

    Saffron has an unusual evidence profile: a botanical that has been tested head-to-head against prescription antidepressants, repeatedly, and held its own. Randomised double-blind trials at 30 mg/day of standardised extract have compared saffron with fluoxetine 20-40 mg and with imipramine 100 mg in mild to moderate depression, finding no significant difference in Hamilton Depression Rating Scale improvement, while saffron produced fewer side effects — notably less sexual dysfunction, which is a common reason people stop SSRIs. Meta-analyses of these trials report a large effect versus placebo and equivalence versus antidepressants. The caveats are substantial and should be stated plainly: nearly all trials come from a handful of Iranian research groups in a saffron-producing country, sample sizes are typically 40-60 participants, durations are six to eight weeks, and independent replication outside that setting is limited. Saffron also has smaller supportive literatures for premenstrual symptoms, sexual dysfunction including SSRI-induced dysfunction, and age-related macular degeneration, where crocin appears to improve retinal function. It is expensive by weight, but the dose is small, and adulteration with safflower or dyed plant material is a real problem in the market.

    Safety Information

    Potential Side Effects

    At 30 mg/day, mild and infrequent: nausea, appetite change, headache, dry mouth. Higher doses cause vomiting, dizziness and, at gram-level doses, serious toxicity including bleeding.

    Contraindications

    Pregnancy, where high doses stimulate uterine contraction. Bipolar disorder without supervision, given the theoretical switch risk of any antidepressant action. Do not use as a substitute for treatment in severe depression.
