    It works, and it has outcome data most supplements lack. The problem is you are taking an unlabelled statin dose without monitoring.

    How It Works

    Monacolin K competitively inhibits HMG-CoA reductase, reducing hepatic cholesterol synthesis and upregulating LDL receptor expression, which clears more LDL from circulation — the same mechanism as any statin.

    Evidence

    What the studies say

    Meta-analyses of randomised trials report LDL cholesterol reductions of approximately 15-25% with red yeast rice preparations, comparable to low-dose statin therapy. The Chinese Coronary Secondary Prevention Study randomised nearly 5,000 patients with prior myocardial infarction to a red yeast rice preparation or placebo and found significant reductions in recurrent coronary events and total mortality over roughly four and a half years — genuine outcome evidence. Against that, independent product analyses have repeatedly found monacolin K content varying from undetectable to over 10 mg per labelled daily dose across commercial brands, and citrinin, a nephrotoxic mycotoxin, in a proportion of samples. The European Food Safety Authority could not establish a safe intake level and EU regulation now restricts monacolin content to under 3 mg per daily dose. Adverse effects mirror statins: myalgia, transaminase elevation and rare rhabdomyolysis.

    Safety

    Caveats

    You are taking a statin at an undeclared dose. Product potency varies enormously, citrinin contamination is documented, and all statin contraindications apply — including pregnancy, liver disease and CYP3A4 inhibitor interactions.
