European food-safety authorities concluded in 2018 that monacolin K at ≥10 mg/day carries the same myopathy and liver-enzyme risks as lovastatin, and product testing repeatedly finds 100-fold variability in monacolin content plus citrinin (a nephrotoxic contaminant) in some brands. The honest framing: this is an unregulated low-dose statin — useful for statin-intolerant patients under medical supervision with liver and CK monitoring, not a side-effect-free alternative.
