The measured benefit is real but moderate, which is why the verdict here is likely rather than certain. Read this page as symptom support with plausible antimicrobial activity, not as a substitute for breath testing or for the antibiotic protocols used when overgrowth is confirmed.

    How It Works

    Menthol blocks calcium channels in intestinal smooth muscle, relaxing spasm and easing transit. Peppermint oil also shows direct antimicrobial activity against several gut organisms in laboratory work, which is the proposed basis for its use around bacterial overgrowth.
    Laboratory antimicrobial activity does not automatically translate into clinical decolonisation at the doses people take. Treat the mechanism as an explanation for symptom relief rather than proof that the bacterial population itself is changing.

    Both analyses agree on direction and roughly on size. The gap in the evidence is population: nobody has run an adequately powered trial in breath-test-confirmed overgrowth, so extrapolation is doing some of the work here.

    Safety

    Heartburn is the commonest problem and the usual reason people stop. Enteric coating reduces it but does not remove it. Peppermint oil is not appropriate for infants, and anyone with gallstones, significant reflux or a hiatus hernia should take medical advice before starting.
