    How It Works

    DHA is the dominant structural fatty acid of photoreceptor outer segments, and EPA-derived resolvins dampen retinal inflammation. The biological rationale is sound; the clinical trials have not borne it out.

    What the studies say

    AREDS2 randomised 1,000 mg of DHA plus EPA daily as a factorial addition to the core formulation and found no reduction in progression to advanced AMD over five years. The earlier NAT2 trial of DHA in early AMD was similarly null on its primary endpoint. This contrasts with consistent observational data linking two or more servings of oily fish weekly to lower AMD incidence, a gap best explained by whole-diet and residual confounding. For dry eye, which is a frequent cause of fluctuating blur, the DREAM trial (2018, n=535) found 3,000 mg of omega-3 no better than olive oil placebo, although earlier smaller trials were positive.

    Caveats

    Doses above 3 g daily may prolong bleeding time and interact with anticoagulants. Fish oil quality varies; rancid product is common and counterproductive.
