Omega-3 Fatty Acids for Raynaud's Disease

Fish oil has a small positive randomised trial in primary Raynaud's showing improved cold tolerance — a plausible adjunct to the mainstays of warmth, smoking cessation, and (when needed) nifedipine.

    How It Works

Omega-3 fatty acids incorporate into blood vessel and platelet membranes, promoting a more vasodilatory prostaglandin profile and reducing the vasospastic tendency of digital arteries.

    Dosing & Protocol

    Typical dose

    Recommended dose
    2-4 g EPA+DHA daily with food
    Expected timeframe
    6-12 weeks to assess

    Protocol

    form
    Triglyceride-form fish oil
    duration
12 weeks through a cold season
    co factor
Take with dietary fat. Evidence is mixed: benefit appeared mainly in primary Raynaud phenomenon, with little effect in secondary Raynaud from connective tissue disease. Cold avoidance, gloves, smoking cessation and stopping beta blockers where possible matter more.
    titration
Increase towards 3000-4000 mg/day over a month - the primary Raynaud trial used a high dose equivalent to about 12 g/day of fish oil, which is impractical and needs supervision
    starting dose
1000 mg combined EPA+DHA daily with a meal

    Evidence

    What the studies say

A randomised controlled trial of fish oil in primary Raynaud's found improved tolerance to cold exposure and delayed onset of vasospasm versus placebo; notably, no benefit appeared in secondary Raynaud's. The evidence base is small — one principal trial plus mechanistic support — so it sits behind the proven levers: cold avoidance, whole-body warming, smoking cessation, and calcium channel blockers with strong trial evidence for frequent attacks.

    Safety

    Caveats

Mild fishy reflux; higher doses have blood-thinning effects — flag before surgery. No evidence in secondary Raynaud's.

    Less likely to help if

People with secondary Raynaud from scleroderma, lupus or vibration injury, where trials showed no benefit.
