    DHA is the dominant fatty acid in neuronal membranes and supports synaptic function; observational data link higher fish intake to lower dementia risk, but that may reflect broader dietary and lifestyle patterns.

    What the studies say

    Large RCTs including the Alzheimer's Disease Cooperative Study DHA trial found no slowing of cognitive decline in established Alzheimer's. Some analyses hint at benefit when started early in cognitively normal people with low omega-3 status, but this is unproven. Blood pressure control, hearing aids, exercise, and social engagement have stronger evidence for risk reduction than any supplement.

    Safety

    Caveats

    Omega-3 does not treat Alzheimer disease and should not displace cholinesterase inhibitors, memantine or the current disease-modifying options. APOE4 carriers appeared to benefit least in trial subgroups. Safety ceiling: stay below 3 g/day EPA+DHA - higher raises bleeding risk, a particular concern with anticoagulants, antiplatelets and in the frail elderly who fall. Stop before surgery. Avoid cod liver oil because of vitamin A. Check for interactions with the full medication list, which is usually long in this group.

    Less likely to help if

    People with established moderate or severe Alzheimer disease, and APOE4 carriers, in whom trials showed no benefit.
