- Evidence is insufficient. Trials of melatonin in perimenopausal and postmenopausal women, typically at 3 mg nightly, have examined sleep quality, mood and bone turnover, and generally report improved sleep - but hot flushes and night sweats have not been shown to reduce. There is no established mechanism linking melatonin to vasomotor symptoms, which arise from a narrowed thermoneutral zone driven by oestrogen withdrawal acting on hypothalamic thermoregulatory and KNDy neuron pathways. Melatonin does lower core body temperature slightly, which is why the hypothesis exists, but that is a different physiological process. Any perceived benefit is likely to reflect better sleep making night sweats less disruptive, plus the large placebo response - routinely 30-50% - seen in menopause trials.
