Overview
L-serine is made in the body from 3-phosphoglycerate, so dietary supply is not usually limiting. Its clinical importance comes from what happens when synthesis fails. In serine biosynthesis defects such as phosphoglycerate dehydrogenase deficiency, high-dose L-serine is an established treatment that can prevent seizures and improve development — a real, guideline-level use. The other notable evidence is in neurodegeneration. A phase I randomized trial in ALS found that 15 g twice daily was tolerable and associated with a slower rate of functional decline than matched historical controls, which is intriguing but not definitive. The interest there stems from the hypothesis that the cyanobacterial toxin BMAA is misincorporated into proteins in place of serine, and that flooding the system with serine is protective — a hypothesis with strong primate data and unfinished human validation. L-serine is also the precursor to glycine, D-serine (an NMDA receptor co-agonist), sphingolipids and phosphatidylserine. For healthy adults with no deficiency, there is no evidence supporting routine use, and doses used in trials are enormous compared with typical supplements.
