    Take with or without food, consistently. Evidence is mixed because the answer depends entirely on iodine status. Iodine is essential for thyroid hormone synthesis, and deficiency causes hypothyroidism and goitre with genuine metabolic consequences - correcting deficiency restores normal thyroid function and metabolic rate, which is well established. In iodine-replete people, additional iodine does not improve thyroid function or metabolism and can actively harm it: excess iodine causes both hypothyroidism (Wolff-Chaikoff effect) and hyperthyroidism (Jod-Basedow phenomenon), and increases the risk of thyroid autoimmunity. Iodine deficiency has re-emerged in some populations through reduced dairy and iodised salt intake, so testing status is worthwhile before supplementing.

    What the studies say

    Deficiency correction restores thyroid function and growth in deficient populations. In iodine-replete adults, supplementation shows no metabolic benefit and can disturb thyroid function.

    Safety

    Caveats

    Safety ceiling and it matters more here than the benefit: the tolerable upper intake level is 1100 mcg (1.1 mg) per day for adults, and many kelp and seaweed supplements exceed this several times over in a single serving. Excess iodine can precipitate both hypothyroidism and hyperthyroidism, and is particularly hazardous in existing autoimmune thyroid disease (Hashimoto or Graves), nodular goitre, and after thyroid surgery or radioiodine. Iodine interacts with lithium, amiodarone (itself iodine-rich), antithyroid drugs and potassium-sparing diuretics. Do not self-treat suspected thyroid disease - fatigue, weight change, cold or heat intolerance and hair loss need TSH testing first, and treatment is levothyroxine where indicated, not iodine. In pregnancy 220 mcg daily is recommended and deficiency causes irreversible harm to fetal brain development, but excess is equally dangerous, so use a pregnancy multivitamin rather than kelp.
