    B vitamins are cofactors throughout carbohydrate and lipid metabolism, and homocysteine lowering has cardiovascular rationale. But metabolic dysfunction is driven by energy balance, adiposity and insulin signalling, none of which are B-vitamin limited.

    What the studies say

    Homocysteine-lowering trials with B vitamins have consistently failed to reduce cardiovascular events despite lowering the marker. No trial shows improvement in HbA1c or insulin sensitivity from B-complex in replete adults. Metformin-associated B12 depletion is well documented and clinically meaningful, affecting a substantial minority of long-term users.

    Caveats

    Do not substitute B vitamins for the interventions that actually move metabolic markers: resistance training, protein-adequate diet, sleep and, where indicated, medication.
