    Evidence is insufficient. This has been tested carefully and repeatedly in healthy adults: randomised trials of standardised ginkgo extract at 120-240 mg/day, including a well-conducted six-week trial in healthy volunteers and multiple meta-analyses, found no improvement in memory formation, learning or recall. The large GEM trial in over 3000 older adults found no effect on memory decline over six years. Whatever modest effect exists in established dementia does not extend to memory enhancement in people whose memory is normal.

    What the studies say

    Randomised trials in healthy volunteers, including a well-known JAMA trial, found no memory benefit over placebo. Positive findings are largely confined to populations with existing cerebrovascular or cognitive impairment.

    EGb761 at 240 mg per day was able to stabilise or slow decline in cognition, function, behaviour and global change at 22-26 weeks in cognitive impairment and dementia.

    Caveats

    What does support memory: sleep (consolidation happens during it), physical exercise, treating hearing loss, blood pressure control, and spaced retrieval practice. New or progressive memory problems need medical assessment for reversible causes such as B12 deficiency, thyroid disease, depression, sleep apnoea and medication effects. Safety: ginkgo has antiplatelet activity, with case reports of intracranial and post-surgical bleeding - avoid with warfarin, DOACs, aspirin, clopidogrel and NSAIDs, and stop at least two weeks before surgery or dental extraction. It may lower the seizure threshold, so avoid in epilepsy. It induces CYP2C19, reducing levels of omeprazole and some antiepileptics, and interacts with efavirenz. Avoid in pregnancy, breastfeeding and children. Use only standardised extracts with ginkgolic acid below 5 ppm; crude leaf products are allergenic.
