    Curcumin has a large randomised literature in pain, particularly knee osteoarthritis, where trials repeatedly report reductions in pain scores that are modest in absolute terms but consistent in direction. Several head-to-head trials against NSAIDs found comparable symptom relief with better gastrointestinal tolerability, which is the most clinically interesting part of the picture. The evidence is weakened by small samples, short durations and frequent industry sponsorship, which is why the verdict is positive rather than definitive.
    Almost everything practical about curcumin comes down to absorption. Plain turmeric powder and unenhanced curcumin extracts are absorbed so poorly that they are unlikely to reproduce trial results, and the trials that worked used piperine-enhanced, phytosome, or other bioavailability-engineered formulations. Expect a gradual effect over four to eight weeks rather than acute relief. Curcumin is a background anti-inflammatory strategy, not a painkiller you take when a joint flares.

    How It Works

    Curcumin's principal action is inhibition of NF-κB, the transcription factor that switches on much of the inflammatory programme. Suppressing it reduces downstream production of TNF-alpha, IL-1beta and IL-6 — cytokines that both drive tissue inflammation and sensitise peripheral nociceptors.
    It also inhibits COX-2 and 5-lipoxygenase, overlapping with the NSAID mechanism but far less potently and without the same degree of COX-1 suppression in gastric mucosa — the proposed reason for its better gastrointestinal profile. Because these are transcriptional and enzymatic effects on an ongoing inflammatory process rather than acute analgesia, benefit accumulates over weeks. Oral curcumin is rapidly glucuronidated and cleared, so formulation determines whether enough reaches tissue to matter; piperine inhibits that glucuronidation, which is why it appears in so many products.

    Dose depends entirely on formulation. A phytosome or piperine-enhanced product at a modest dose can outperform several grams of plain extract. Take with a fat-containing meal and give it at least eight weeks before judging.

    Safety

    Curcumin is generally well tolerated. Nausea, loose stools and reflux are the usual complaints, more common at higher doses and on an empty stomach, and typically settle with food or a lower dose.

    Bleeding risk, gallbladder disease and liver reports

    Curcumin has mild antiplatelet activity: stop it at least two weeks before surgery and use caution with anticoagulants or antiplatelet drugs. It stimulates gallbladder contraction, so avoid it with gallstones or bile duct obstruction. Rare cases of drug-induced liver injury have been reported, mostly with high-dose enhanced formulations — stop and seek advice if you develop jaundice, dark urine or unexplained fatigue.
