    The influential idea in this field is not collagen alone but collagen plus load. The proposed protocol pairs hydrolysed collagen with vitamin C, taken about an hour before a short bout of loading, on the reasoning that blood amino acid levels peak while the tendon is being stimulated to remodel. The evidence base is small and biomarker-heavy. Studies commonly measure collagen synthesis markers or pain scores rather than tendon strength directly, so this remains a promising rather than a proven intervention.

    How It Works

    Hydrolysed collagen is digested into di- and tripeptides, notably prolyl-hydroxyproline, which appear intact in the bloodstream. These fragments supply the glycine, proline and hydroxyproline that make up the repeating collagen triple helix.
    Beyond raw material, these peptides appear to act as signals: in cell work they stimulate fibroblast and tenocyte proliferation and upregulate collagen gene expression, so the effect may be partly informational rather than purely substrate-driven. Vitamin C is non-negotiable in this pathway because prolyl and lysyl hydroxylases require ascorbate as a cofactor to hydroxylate proline and lysine residues. Without hydroxylation the triple helix is unstable, which is why collagen and vitamin C are dosed together.

    Collagen without progressive tendon loading has no plausible route to increasing tendon strength. Heavy slow resistance and isometrics remain the primary treatment.

    The wider literature contains a handful of small trials, mostly in young active adults, using gelatin or collagen peptides with vitamin C before loading and reporting increased circulating markers of collagen synthesis such as amino-terminal propeptide of type I collagen. Several limitations should temper conclusions: samples are typically fewer than thirty participants, most measure blood markers rather than tendon mechanical properties, imaging outcomes are inconsistent, and adequately powered trials with strength or injury endpoints do not yet exist.

    Synthesis markers, not strength

    A rise in a blood marker of collagen synthesis is not the same as a measurably stronger tendon. That step has not been demonstrated.

    Safety

    Hydrolysed collagen has a strong safety record. Mild bloating, fullness or an unpleasant taste are the usual issues, and it is essentially a protein food.

    Rehab first, supplement second

    Tendinopathy responds to progressive loading under a physiotherapist. Adding collagen while skipping the rehab programme wastes months of slow-healing tissue time.

    Marine collagen is unsuitable for anyone with a fish or shellfish allergy, and bovine collagen may not fit religious or dietary requirements. Collagen is an incomplete protein lacking tryptophan, so it should supplement rather than replace dietary protein. Acute tendon pain following a snap or pop needs medical assessment for rupture, not supplementation.
