    Hydrolyzed collagen provides glycine and proline for tendon matrix synthesis; small RCTs show increased collagen synthesis rates when taken ~1 hour before loading exercise, particularly with vitamin C as a cofactor for hydroxylation.

    What the studies say

    A handful of small RCTs (notably in patellar and Achilles tendinopathy) show modest improvements in pain and function with collagen plus exercise versus exercise alone. Effect sizes are small and studies are short; the loading program does most of the work.

    Caveats

    Verdict is mixed — several loading trials found no added benefit from collagen over loading alone. Collagen does nothing without mechanical loading. Acute rupture, or tendon pain during fluoroquinolone antibiotic use, needs urgent medical review. Fish allergy risk with marine collagen. Not studied in pregnancy.

    People not doing progressive loading, those with full-thickness tears, inflammatory arthritis, or corticosteroid-injected tendons.
