    Insufficient evidence

    B-complex does not treat anxiety in people with normal B-vitamin status. It helps only where deficiency (B12, folate, B6) is part of the picture.

    How It Works

    B6 is a cofactor for the decarboxylation step that makes serotonin and GABA; B12 and folate drive methylation and homocysteine clearance. When any of these run low, monoamine synthesis and methylation capacity fall, which can present as irritability, low mood and anxious arousal. Restoring status normalises those pathways — it does not push them above normal.

    Evidence

    What the studies say

    Trials of high-dose B-complex in unselected stressed adults show small reductions in self-reported strain and anxiety over 4-12 weeks, with the largest effects in participants who started with poor dietary intake. In people with confirmed B12 or folate deficiency, correction produces clear symptomatic improvement. Head-to-head against established anxiety treatments, B vitamins are not competitive.

    Safety

    Caveats

    B6 above 50 mg/day taken long term can cause peripheral neuropathy. High-dose niacin can cause flushing. B vitamins turn urine bright yellow — harmless.

    Less likely to help if

    People with adequate B-vitamin status, and anyone whose anxiety is driven by sleep debt, caffeine, thyroid disease or an untreated anxiety disorder.
