    How It Works

    Withanolides appear to modulate GABA-A receptor signalling and dampen hypothalamic-pituitary-adrenal axis output, lowering cortisol response to stressors. This reduces the physiological arousal component of anxiety rather than the cognitive appraisal that drives social fear.

    Evidence

    What the studies say

    Multiple small randomised placebo-controlled trials, mostly 6-8 weeks with 300-600 mg daily of root extract, report reductions in Hamilton Anxiety and Perceived Stress Scale scores alongside lower morning cortisol. Trial quality is a persistent problem: sample sizes are typically under 80, many are industry-funded, and Indian single-centre studies dominate. Crucially, the populations studied are people with stress or generalised anxiety, not diagnosed social anxiety disorder, and none measured the Liebowitz Social Anxiety Scale. Since social anxiety is maintained by self-focused attention and safety behaviours rather than by baseline arousal alone, reducing physical arousal may reduce blushing and trembling without altering avoidance. It may make exposure work more tolerable, but it does not replace it.

    Caveats

    Rare but documented cases of liver injury have led several regulators to require warnings. Avoid in pregnancy, in autoimmune thyroid disease since it raises thyroid hormone levels, and alongside immunosuppressants or sedatives.

    People whose social anxiety is driven by cognitive fear of evaluation rather than physical arousal symptoms.
