Overview
A noticeable loss of strength for everyday tasks — opening jars, climbing stairs, rising from a chair. After 50, gradual muscle loss (sarcopenia) is common but reversible with resistance training; weakness that arrives quickly, affects one side, or comes with other symptoms is a different and more urgent picture.
Root Causes
Evidence synthesis: Progressive resistance training is the highest-evidence intervention for age-related weakness — trials consistently show strength gains even in the very elderly, with protein intake (1.0-1.2 g/kg) supporting the response. Creatine monohydrate has a large trial base showing modest additional strength and lean-mass gains when combined with resistance training in older adults. Vitamin D supplementation improves muscle function only in the deficient. The triage rules are clinical: sudden focal weakness is stroke territory (FAST); progressive symmetrical weakness in younger adults warrants thyroid function, anaemia, B12, glucose, and electrolyte testing; weakness with dark urine after exercise or statins suggests muscle breakdown needing urgent review.
