Overview
Hypervigilance is a sustained state of heightened threat monitoring — sitting facing the door, tracking exits, startling violently at sudden noises, scanning faces for signs of anger, and being unable to settle even in objectively safe environments. It is exhausting in a way that is easy to underestimate, because the brain is running continuous background threat assessment that consumes attention, disrupts sleep and depletes cognitive resources. It is a core symptom of post-traumatic stress disorder, where it forms part of the hyperarousal cluster alongside irritability, sleep disturbance and concentration problems, and it appears in generalised anxiety disorder, panic disorder, and after any significant threat exposure. Importantly, hypervigilance is a learned protective adaptation, not a character flaw — the nervous system calibrated itself to a genuinely dangerous environment and has not recalibrated afterwards. This framing matters clinically, because attempts to simply relax or think rationally rarely touch it; the response operates below conscious control through amygdala-driven threat circuits. The effective treatments are trauma-focused psychological therapies — trauma-focused CBT and EMDR have the strongest evidence — sometimes with SSRIs, and prazosin specifically for trauma nightmares. Substances that seem to help acutely, particularly alcohol and benzodiazepines, consistently worsen the underlying condition.

Root Causes

Sleep deprivation from the resulting insomnia further impairs prefrontal control, creating a self-reinforcing loop. Because the learning is implicit and fear-conditioned, it does not respond to logical reassurance — which is why exposure-based and trauma-processing therapies, which work at the level of the conditioned response, outperform talking about the problem.
