Overview
Heart failure means the heart fails to pump sufficient blood for the body's demands — it does not mean the heart has stopped. In the reduced-ejection-fraction form, the weakened left ventricle pumps out a smaller fraction of its contents with each beat; in the preserved form, the stiffened ventricle fills poorly despite a normal ejection fraction. Either way, pressure backs up: fluid accumulates in the lungs causing breathlessness on exertion, lying flat or at night, and in the legs causing ankle swelling, while fatigue reflects under-supplied muscles. Coronary artery disease and previous heart attacks are the leading cause, followed by long-standing hypertension, valve disease, diabetes, atrial fibrillation and, less commonly, alcohol, chemotherapy and viral myocarditis. Treatment has been transformed: the four-pillar drug regimen — an ARNI or ACE inhibitor, a beta-blocker, an MRA and an SGLT2 inhibitor — each independently reduces death and hospitalisation in reduced-ejection-fraction disease, and cardiac devices help selected patients. Daily weight monitoring catches fluid retention days before symptoms. Salt and fluid discipline, vaccination, and cardiac rehabilitation complete the picture. Supplements must be handled carefully in heart failure; several common ones interact with its medications.

Root Causes
The failing heart triggers compensatory systems that become part of the disease: the renin-angiotensin and sympathetic systems retain salt and water and constrict vessels, maintaining pressure short-term while overloading the ventricle long-term. The modern drug pillars work by blocking these compensations. Structural damage from infarction, pressure overload from hypertension, volume overload from leaking valves and direct toxicity from alcohol or chemotherapy each initiate the same final pathway of remodelling and decline.

Diagnostic Markers
- Echocardiogram measuring ejection fraction — the central test
- BNP or NT-proBNP blood testing, with high levels confirming and low levels largely excluding heart failure
- ECG for rhythm, ischaemia and conduction disease
- Renal function and electrolytes, shaping medication choices
- Iron studies and full blood count, since iron deficiency worsens outcomes and is treatable
- Chest X-ray for lung congestion
- Coronary assessment where ischaemia is suspected
- Daily home weight as an ongoing monitoring marker
When to See a Doctor
Seek urgent care for new severe breathlessness, breathlessness at rest, waking gasping, chest pain, fainting, or rapid weight gain of more than 2 kg in three days with worsening swelling — these signal decompensation needing same-day assessment. Any new exertional breathlessness or ankle swelling deserves prompt review, because earlier treatment preserves heart function. If you have established heart failure, report weight trends rather than waiting for symptoms.
Supplements Studied For This
D-Ribose
Small crossover trials found improved diastolic function and better quality-of-life scores with 15 g per day in chronic heart failure, and the ATP-depletion mechanism is coherent. But no trial has enrolled more than about 40 patients, none has measured hospitalisation or mortality, and the work has not advanced in over a decade.
Hawthorn
Improves symptoms and exercise capacity as an add-on, but does not improve survival. Strictly adjunctive to guideline-directed therapy.
CoQ10
CoQ10 has the best evidence of any supplement in heart failure: the Q-SYMBIO trial showed reduced major cardiac events and mortality over two years as an add-on to standard therapy. It remains an adjunct, never a substitute for the four medication pillars.

Suggested Pattern
Salt discipline is the dietary cornerstone: sodium drives fluid retention, and limiting intake toward 2 g of sodium (5 g of salt) daily reduces congestion and hospitalisation. Most dietary sodium hides in bread, processed meats, cheese, tinned goods and restaurant food rather than the salt shaker. Fluid restriction applies only in selected advanced cases under guidance. Adequate protein and calorie intake matter because cardiac cachexia — wasting in advanced disease — worsens outcomes. Alcohol should be eliminated where it caused or contributes to the cardiomyopathy and minimised otherwise. Potassium balance requires care, since several heart failure drugs raise it.
Eat more
- Fresh, home-cooked food where you control the salt
- Adequate protein at each meal to protect muscle mass
- Potassium-rich foods only as advised — several heart failure drugs already raise potassium
- Oily fish as part of a cardioprotective pattern
- Small frequent meals if fullness and poor appetite feature
Avoid
- Processed and packaged foods — the dominant sodium source
- Cured meats, smoked fish, cheese, pickles and stock cubes
- Adding salt at the table or in cooking
- Excess alcohol; total abstinence where alcohol caused the cardiomyopathy
- Large fluid volumes in advanced disease where restriction is advised
- Grapefruit with certain cardiac medications — check your specific drugs
