Overview
Oedema becomes visible only after several litres of interstitial fluid have accumulated, which is why swelling is a late sign. Distribution is the diagnostic clue: symmetrical ankle swelling that worsens through the day suggests venous or gravitational causes; swelling with breathlessness on lying flat suggests the heart; puffiness around the eyes on waking suggests kidney protein loss; asymmetrical swelling of one limb with pain suggests clot or lymphatic obstruction. Low albumin from liver disease or malnutrition produces soft, widespread oedema. Calcium channel blockers cause a very common and entirely benign drug-related ankle oedema that is frequently misattributed to the heart.

Root Causes
Increased capillary hydrostatic pressure (heart failure, venous insufficiency), reduced oncotic pressure (nephrotic syndrome, liver disease), increased permeability (inflammation, sepsis), or lymphatic obstruction.
How It's Diagnosed
Diagnostic Markers
- Daily weight and fluid balance
- Urine albumin-to-creatinine ratio
- Serum albumin, creatinine and eGFR
- Liver function tests
- NT-proBNP if cardiac cause is suspected
- Doppler ultrasound for one-sided leg swelling
When to See a Doctor
Seek urgent care for sudden one-sided leg swelling with pain, breathlessness at rest or on lying flat, chest pain, or facial and tongue swelling. Arrange prompt review for rapid weight gain, frothy urine, or new widespread oedema.
