Overview
Salicin from willow bark is hydrolysed by gut flora and hepatic metabolism into salicylic acid, the same active metabolite aspirin produces. The pharmacology differs in useful ways: conversion is slower, plasma peaks are lower and later, and willow bark does not irreversibly acetylate platelet COX-1, so antiplatelet effects are weaker. This explains the better gastric tolerability reported in trials. Randomized studies of extracts standardized to 120-240 mg salicin per day have shown pain reduction in chronic low back pain, with the higher dose clearly outperforming placebo, and smaller studies suggest benefit in osteoarthritis of the knee and hip. Willow bark extract also contains polyphenols and flavonoids that may contribute independently, since the salicin content alone does not fully account for effect size. The important caution is that it is still a salicylate: aspirin allergy, anticoagulation, pediatric fever with viral illness (Reye syndrome concern) and peptic ulcer are all genuine contraindications.
