    How It Works

    Dopaminergic action at the pituitary lowers prolactin and modulates the luteal hormone environment — relevant to perimenopausal fluctuation, less so once cycles have ceased.

    Evidence is insufficient and the mechanism argues against use here. Vitex acts on dopamine D2 receptors in the pituitary, reducing prolactin secretion and thereby modestly supporting luteal phase progesterone - a mechanism that depends on having ovulatory cycles. After menopause there is no cycle to support, and the trials supporting vitex are in premenstrual syndrome, cyclical mastalgia and luteal phase defects, not menopause. The few combination products tested for menopausal symptoms include vitex alongside other herbs, making attribution impossible.

    Strong RCT support exists for PMS and cycle irregularity; direct menopause trials are small and mixed, with one combination-product trial showing benefit that cannot be attributed to vitex alone.

    Caveats

    May alter cycle timing — expect 2–3 cycles for effects; caution with dopaminergic medications and hormonal contraception; discontinue if pregnant.

    Less likely to help if

    Fully postmenopausal women with purely vasomotor symptoms are unlikely to respond.
