Liu and colleagues randomised middle-aged adults to urolithin A for 4 months and found improved muscle endurance measured as the number of repetitions to fatigue in hand and leg muscles, alongside reduced inflammatory biomarkers - but no significant improvement in the primary endpoint of peak power or aerobic capacity. A subsequent trial in older adults similarly found improved muscle strength and endurance measures without change in VO2max or six-minute walk distance. So the effect appears real but narrow, confined to local muscle endurance rather than whole-body performance.

    Caveats

    The pattern across urolithin A trials is consistent: secondary muscle endurance endpoints improve while primary endpoints of peak power and aerobic capacity do not, which means benefit is narrow and should not be expected to show up in running, cycling or general fitness. Nearly all trials are funded by the ingredient developer. Training itself remains overwhelmingly the determinant of endurance, alongside adequate carbohydrate and iron status - and among supplements, nitrate from beetroot juice and caffeine have stronger performance evidence. Safety of urolithin A: well tolerated at 500-1,000 mg/day over 4 months in trials, with no significant adverse effects and FDA GRAS status, though no upper limit is established and long-term data beyond about a year are absent. Mild gastrointestinal upset is occasionally reported. Interactions have not been systematically studied. Safety in pregnancy and breastfeeding is not established. Competitive athletes should use third-party certified products. Unexplained exercise intolerance or breathlessness disproportionate to fitness needs medical assessment for anaemia, cardiac or respiratory causes.
