    Saw Palmetto for DHT Balance

    Saw palmetto modestly inhibits 5-alpha-reductase; meta-analyses show small but real improvements in urinary symptoms and hair outcomes versus placebo.

    Overview

    Saw palmetto is widely marketed as a natural DHT blocker, and it does inhibit 5-alpha reductase in laboratory work. The practical question is how much that translates into measurable change in humans, and the honest answer is far less than finasteride. In a two-year comparison in male androgenetic alopecia, finasteride outperformed Serenoa repens, though a minority of saw palmetto users did show improvement.
    Recent systematic reviews of supplements for hair loss place saw palmetto among the better-supported botanicals while stressing that evidence quality is low and trials are small. The reasonable framing: a modest, mainly local effect with a much gentler side-effect profile than pharmaceutical 5-alpha reductase inhibitors, which is why some people choose it despite the weaker effect.

    How It Works

    Testosterone is converted to the more potent androgen dihydrotestosterone by 5-alpha reductase, present as type 1 in sebaceous glands and scalp and type 2 in prostate and hair follicles. DHT drives follicular miniaturisation in androgenetic alopecia and stromal growth in the prostate. Saw palmetto lipid extracts inhibit both isoenzymes in vitro, unlike finasteride, which is type 2 selective.
    The inhibition is non-competitive and considerably weaker than pharmaceutical inhibition. Finasteride reduces serum DHT by roughly 70 percent; saw palmetto produces little consistent change in circulating DHT in human studies. That gap suggests any real effect is local tissue activity rather than systemic androgen suppression, and it explains why clinical results are smaller and less reliable.

    Dosing & Protocol

    The standard oral dose is 320 mg per day of liposterolic extract standardised to 85 to 95 percent fatty acids and sterols, taken as one dose or split into two. This is the dose used in prostate and hair trials alike. Topical formulations at 2 to 5 percent have been studied for scalp application. Hair outcomes are slow: give any intervention at least six months before judging.

    Safety

    Saw palmetto is well tolerated, with adverse event rates in trials close to placebo. Mild gastrointestinal upset is the usual complaint and is reduced by taking it with food. Sexual side effects are reported far less often than with finasteride. Rare cases of liver and pancreatic injury have been described. Women who are pregnant or may become pregnant should avoid it because of theoretical anti-androgenic effects on a male fetus.

    Avoid in pregnancy

    Anti-androgenic activity makes saw palmetto unsuitable in pregnancy or when trying to conceive. Men planning conception should also discuss it with a clinician.

    Interactions & Conflicts

    The clinically relevant conflicts are hormonal and haematological. Combining saw palmetto with finasteride or dutasteride adds anti-androgenic load without evidence of extra benefit, and it may interact with hormonal contraceptives or hormone therapy through the same androgen pathway. Case reports describe increased bleeding, so it should be stopped before surgery and used cautiously with anticoagulants.
