Red yeast rice reliably lowers LDL cholesterol by 15-25%, but its effect on triglycerides is small and inconsistent across trials - meta-analyses report modest reductions of roughly 10-20 mg/dL, with wide confidence intervals and considerable heterogeneity. This mirrors statins generally: they are LDL drugs, with only moderate triglyceride effects that are most apparent when baseline triglycerides are high. For substantially elevated triglycerides, the effective agents are different - fibrates, high-dose omega-3, and above all reduced alcohol, reduced refined carbohydrate and weight loss.
