    No controlled trials support probiotics for tongue coating. If coating is actually oral candidiasis (white patches that scrape off leaving redness), antifungal treatment — not probiotics — is the answer. Persistent coating warrants a dental or medical exam.

    Caveats

    A thick white coating that does not scrape off, or that is painful, may be oral candidiasis or leukoplakia - both need clinical assessment, and leukoplakia can be premalignant. Recurrent oral thrush warrants checks for diabetes, inhaled steroid technique and immunosuppression. Avoid live probiotics in severe immunosuppression or with central venous catheters. Generally acceptable in pregnancy; confirm with a clinician. Persistent tongue changes with a smoking or alcohol history need dental or ENT review.
