    Overview

    Probiotics for infection prevention is a claim that deserves precision. The trial data support a modest reduction in the number of people who get certain common infections, chiefly upper respiratory and recurrent urinary tract infections, when a well-characterised strain is taken daily for months. What the data do not support is broad protection against bacterial infection in general. Benefit is strain-specific and site-specific, so matching the product to the studied strain matters more than the size of the number on the label.

    Fewer infection episodes and less antibiotic use in some populations, with low to moderate certainty.

    How It Works

    Selected Lactobacillus and Bifidobacterium strains compete with pathogens for adhesion sites and nutrients, produce organic acids and bacteriocins, tighten epithelial junctions, and prompt secretory IgA responses at the mucosal surface.
    These effects are strain-dependent and largely transient. Most supplemented organisms do not colonise permanently, which is why continuous daily dosing is what the prevention trials tested.

    Both reviews find benefit, and both flag the same weaknesses: small trials, varied strains and inconsistent reporting. That combination supports a likely verdict rather than a strong one.

    Safety

    Healthy adults tolerate probiotics well, with transient bloating or wind in the first week. The meaningful caution is immunocompromise: in critically ill, post-transplant or catheterised patients there are case reports of probiotic organisms causing bloodstream infection.

    Not for everyone

    Avoid unsupervised use if you are immunocompromised, seriously ill, or have a central venous catheter.
