    Take on an empty stomach in the morning; taking it later can interfere with sleep. Evidence is mixed and thin. Small randomised trials in myalgic encephalomyelitis/chronic fatigue syndrome report modest reductions in fatigue scores with 10-20 mg/day, and a trial combining NADH with coenzyme Q10 showed improvement in fatigue and sleep measures. But the studies are small, short, inconsistently blinded and heterogeneous in how fatigue was measured, and larger independent replication is absent. Mechanistically NADH is the reduced form of nicotinamide adenine dinucleotide and the direct electron donor to complex I of the mitochondrial electron transport chain, so a bioenergetic rationale exists - but oral bioavailability is poor and how much reaches mitochondria is unclear.
