    Vivid dreams are a commonly reported adverse effect in melatonin trials and case series, especially above 3–5 mg. For vivid dreams from other causes — stress, antidepressants, REM rebound after sleep deprivation — the answer is addressing the trigger. New dream-enactment behavior (acting out dreams) in adults over 50 warrants neurologic evaluation.

    Caveats

    Melatonin can cause vivid dreams rather than relieve them - if they started after beginning it, reduce or stop. Safety ceiling: doses above 5 mg add grogginess and dream intensity without benefit. Interacts with warfarin, fluvoxamine, benzodiazepines and immunosuppressants. Not established in pregnancy or breastfeeding. Do not drive within five hours. Dream enactment with movement may signal REM sleep behaviour disorder and needs specialist review.

    People whose vivid dreams stem from SSRIs, alcohol withdrawal, PTSD or REM behaviour disorder.
