Melatonin for Nightmares
Melatonin is more likely suspect than solution for nightmares — vivid dreams and nightmares are recognised side effects, particularly above 3-5 mg, because it increases REM sleep where nightmares occur.

Dosing & Protocol
Typical dose
- Recommended dose
- If used for sleep: 0.5-3 mg; avoid high doses if nightmares occur
- Expected timeframe
- Nightmare changes would appear within days of dose adjustment

- Evidence is insufficient: melatonin has not been shown to reduce nightmares, and because it increases REM density it can make vivid dreaming more frequent in some people. The trial doses of 0.5-5 mg before bed were tested for sleep timing, not dream content. Image rehearsal therapy and, in PTSD, prazosin are the interventions with evidence.

Shah C, Kablinger A
Shortly after initiation of ramelteon, he described vivid nightmares leading to discontinuation of ramelteon. The nightmares ameliorated with ramelteon discontinuation.
Safety
Caveats
Vivid dreams and nightmares are dose-related side effects. If nightmares persist after stopping, look to stress, alcohol, and other medications as drivers.
