Overview
Hesperidin illustrates how differently the same compound can be regarded on either side of a regulatory border. In the United States it is a citrus bioflavonoid sold with vague antioxidant claims; in France, Italy and much of Europe the micronised purified flavonoid fraction of 90% diosmin and 10% hesperidin is a licensed medicine for chronic venous disease and haemorrhoids, and appears in clinical guidelines. The evidence supports the European view. A Cochrane review of phlebotonics found benefit for oedema and for symptoms of chronic venous insufficiency, and trials in acute haemorrhoidal disease show faster resolution of bleeding, pain and inflammation, with the combination also used to reduce recurrence and post-haemorrhoidectomy symptoms. Micronisation matters: reducing particle size below two micrometres substantially increases absorption of these poorly soluble flavonoids, and the trial evidence uses the micronised product. Hesperidin also has a smaller literature on endothelial function and blood pressure, where effects are modest, and its metabolite hesperetin is a plausible contributor to the vascular benefits attributed to citrus intake generally. Ordinary non-micronised hesperidin capsules are considerably less well supported.

Potential Side Effects
Generally mild: gastrointestinal upset, nausea, diarrhoea and headache, mostly at the higher acute doses. Well tolerated at maintenance doses.
Contraindications
Undiagnosed rectal bleeding requires medical assessment rather than self-treatment. Avoid in the first trimester of pregnancy.
