    Not recommended as a first choice. Spend the money and effort on strengthening and weight management, which have far stronger evidence.

    How It Works

    Glucosamine is a substrate for glycosaminoglycan synthesis in cartilage, and the theory was that oral supplementation would support cartilage matrix repair. Human pharmacokinetics undermine this: joint concentrations after oral dosing are far below those used in cell culture experiments.

    Evidence

    What the studies say

    The Glucosamine/Chondroitin Arthritis Intervention Trial randomised 1,583 patients and found neither glucosamine 1500mg, chondroitin 1200mg, nor the combination beat placebo for the primary outcome; a subgroup with moderate-severe pain showed a signal that did not survive later scrutiny. A large BMJ network meta-analysis concluded the effect on pain is clinically irrelevant. Industry-funded trials of glucosamine sulfate report better results than independent ones, a pattern that itself argues against a real effect.

    In 147 patients with grade I-II knee osteoarthritis, adding methylsulfonylmethane to glucosamine-chondroitin sulfate for 12 weeks produced greater improvement in WOMAC pain and function scores than glucosamine-chondroitin alone, though the between-group difference was modest and both arms improved substantially.

    Safety

    Caveats

    Derived from shellfish in most products, so a concern with shellfish allergy. May modestly affect blood glucose. Interacts with warfarin in case reports.

    The trial evidence suggests most people, including those with mild to moderate knee osteoarthritis.
