    Overview

    CoQ10 has become a routine part of fertility clinic advice for women with diminished ovarian reserve or advanced maternal age, which is unusual for a supplement. The rationale rests on a specific and well-described feature of egg biology.
    The oocyte contains more mitochondria than any other human cell, and the energy those mitochondria supply drives meiotic spindle assembly and accurate chromosome separation. Mitochondrial function declines with age, and that decline is one proposed contributor to the rise in aneuploidy and the fall in egg quality after the mid-thirties. What can be said honestly is that the biology is compelling and the human trial data is limited. Reported effects concern retrieved egg numbers and fertilisation rates more than live births.

    How It Works

    CoQ10 carries electrons between complexes I/II and III in the mitochondrial electron transport chain. Oocyte maturation, spindle formation and the first cell divisions after fertilisation are among the most ATP-demanding processes in human biology, and the egg must supply that energy from mitochondria it inherited long before.
    Endogenous CoQ10 synthesis falls with age, and animal work in aged mice has shown that CoQ10 supplementation restores mitochondrial function in oocytes and improves ovulation rate and egg quality - the most persuasive strand of evidence in this area. As ubiquinol it also acts as a lipid-soluble antioxidant in the follicular fluid environment, limiting oxidative damage to the oocyte membrane and to mitochondrial DNA, which lacks the repair machinery protecting nuclear DNA.

    Start early or not at all

    A follicle takes roughly three months to mature. Beginning CoQ10 in the month of an IVF cycle leaves no time to influence the eggs being retrieved.

    Beyond this pairing, small randomised trials in women with poor ovarian response undergoing IVF have reported higher numbers of retrieved oocytes, more high-quality embryos and improved fertilisation rates after CoQ10 pretreatment. The aged-mouse work supporting the mechanism is strong. The qualifications are substantial: trials are small, unblinded in some cases, restricted to poor-responder and older populations, and inconsistent in dose and duration. Live birth rate - the endpoint patients care about - is rarely reported and no adequately powered trial has demonstrated an improvement in it.

    Safety

    CoQ10 is well tolerated even at fertility doses, with mild nausea, appetite loss or insomnia from late dosing the usual complaints.

    Safety of high-dose CoQ10 in early pregnancy is not established, so continuation after a positive test should be a clinical decision. Fertility difficulties warrant investigation of both partners - ovulatory disorders, tubal factors, endometriosis, thyroid disease and male factor are all common and often treatable causes that supplements do not address.
