    Overview

    Undenatured type II collagen is the most mechanistically interesting joint supplement, because it works in a way that has nothing to do with the usual "supplies the raw materials" logic. The dose is the giveaway: 40 mg a day, roughly a thousandth of the ten grams of hydrolysed collagen peptides people take for skin. That is because UC-II is not absorbed as amino acids for cartilage synthesis — it interacts with Peyer's patches in the small intestine, where intact type II collagen epitopes induce oral tolerance, expanding regulatory T cells that migrate to joints and downregulate the inflammatory response against native cartilage collagen. Denaturing the protein destroys the epitopes and the effect, which is why processing matters absolutely and why hydrolysed collagen is a different product entirely. Clinically, a randomised trial in knee osteoarthritis compared UC-II 40 mg against glucosamine 1,500 mg plus chondroitin 1,200 mg and found greater WOMAC improvement with UC-II, and further trials in both osteoarthritis and exercise-related joint discomfort in healthy adults point the same way. The evidence base is smaller than that for glucosamine and heavily concentrated among trials involving the ingredient manufacturer, which is the main reason for caution. Onset takes around three months.

    Potential Side Effects

    Very well tolerated at 40 mg. Occasional mild gastrointestinal upset, headache or constipation, at rates close to placebo in trials.

    Contraindications

    Chicken allergy, given the usual source is chicken sternal cartilage. Caution in autoimmune disease where immune modulation is already being managed pharmacologically.
