    Overview

    Betaine, also called trimethylglycine, was first isolated from sugar beet and is found in beetroot, spinach, quinoa and wheat bran. Typical dietary intake is 100-400 mg/day, well below supplemental doses. Its two roles are distinct: it donates a methyl group to convert homocysteine back to methionine, and it acts as an organic osmolyte that helps cells maintain volume under stress.

    The homocysteine effect is the best documented. Doses of 3-6 g/day lower plasma homocysteine reliably, and betaine anhydrous is an approved treatment for inherited homocystinuria. Whether that lowering reduces cardiovascular events is unresolved, and higher doses can raise LDL cholesterol, which complicates the picture.

    In sport, roughly a dozen small trials at 2.5 g/day have examined strength, power and body composition in resistance-trained men. Results are mixed but lean positive for lean mass and repeated-effort work, with less consistent effects on one-rep max. Effect sizes are small, and most trials ran six to eight weeks. Betaine also supports hepatic fat export via phosphatidylcholine synthesis, which is why it appears in fatty liver research.

    Potential Side Effects

    Generally well tolerated. Gastrointestinal upset, diarrhoea and a fishy body odour at higher doses. The clinically relevant effect is a rise in LDL cholesterol at doses above about 4 g/day.

    Contraindications

    Higher doses raise LDL cholesterol and total cholesterol in some trials, so use caution if lipids are already elevated. Use in kidney disease should be supervised.
