    Vitamin B12 for Nerve Pain Relief

    B12 repletion resolves or halts deficiency neuropathy, but adds nothing when levels are already adequate.

    Overview

    Vitamin B12 relieves nerve pain when, and only when, the pain is caused by B12 deficiency. Repletion halts progression and often resolves the burning, tingling and numbness of deficiency neuropathy. In people with normal B12, it adds nothing.

B12 is sold widely as a general nerve supplement, but the evidence for pain relief in chemotherapy-induced, alcohol-related or compressive neuropathy is weak to absent, and those causes need their own treatment. The practical step is testing before treating. Serum B12 in the low-normal range is ambiguous; methylmalonic acid resolves it, and folate supplementation can normalise the blood count while nerve damage quietly continues.

    How It Works

    B12 is required for myelin maintenance and for methylmalonyl-CoA metabolism. Deficiency causes accumulation of methylmalonyl-CoA and abnormal fatty acids in myelin, producing demyelination of peripheral nerves and the dorsal columns of the spinal cord.
    Demyelinated fibres fire abnormally, which is the source of the burning, pins and needles and electric sensations. Restore the cofactor and remyelination can proceed, which is why symptom recovery follows repletion over weeks to months rather than immediately. It also explains why late treatment disappoints. Once axonal loss has replaced demyelination, the substrate for recovery is gone.

    Dosing & Protocol

    For documented deficiency, 1000 mcg of oral B12 daily is effective for most people, including many with pernicious anaemia, because passive diffusion works independently of intrinsic factor at that dose. Intramuscular loading is preferred where malabsorption is severe or neurological signs are advancing. Recheck B12 and methylmalonic acid at three months.

    Safety

    Oral B12 is remarkably safe. It is water soluble, excess is excreted, and no upper intake level has been set. Injections can cause local reactions and, rarely, acneiform eruptions. Cyanocobalamin is avoided in Leber hereditary optic neuropathy, where hydroxocobalamin is used instead.

    Do not start folate or a high-folate multivitamin before B12 status is known. Folate can correct the anaemia while the neurological damage progresses undetected.
