    Evidence

    What the studies say

    Pooled diabetic neuropathy trials at 1500-3000 mg daily report reduced pain scores and some nerve fibre regeneration. In contrast, a large randomised trial in taxane chemotherapy found ALCAR increased neuropathy, so it should not be used prophylactically during chemotherapy.

    In two 52-week trials, 1000 mg three times daily improved pain, vibratory perception and sural nerve fibre regeneration, while 500 mg was largely ineffective.

    Caveats

    Do not use during taxane chemotherapy; may cause fishy body odour.

    Less likely to help if

    Chemotherapy-induced neuropathy (where it can worsen symptoms), compressive or entrapment neuropathies, and long-standing severe axonal loss with no remaining fibres to regenerate.
