    How It Works

    B12 is a cofactor for methionine synthase and methylmalonyl-CoA mutase, required for myelin maintenance and haematopoiesis. Deficiency causes macrocytic anaemia and neurological dysfunction, both of which present as fatigue.

    Evidence

    What the studies say

    Trials of B12 in people with normal serum levels show no improvement in energy or cognitive performance over placebo. In documented deficiency, replacement reliably resolves associated fatigue and prevents irreversible neurological damage. Serum B12 is an imperfect marker; methylmalonic acid and homocysteine are more sensitive when the result is borderline (148-258 pmol/L). Metformin, proton pump inhibitors, and a strict plant-based diet without fortification are the common causes worth checking for.

    No studies are yet linked to both Vitamin B12 and Morning Fatigue.

    Caveats

    Do not treat B12 deficiency without also checking folate — correcting one alone can mask or worsen the other. High-dose B12 is otherwise safe.
