    What the studies say

    Trials of B12 for fatigue in non-deficient people show no consistent benefit. In contrast, deficient patients report marked energy improvement within weeks of repletion. Borderline levels (200-400 pg/mL) with elevated MMA or homocysteine identify the grey-zone patients most likely to respond.

    Caveats

    Fatigue has dozens of causes — iron, thyroid, sleep apnea, depression. B12 is one line of investigation, not a universal energy pill. Avoid megadose B-complex marketing claims.

    Less likely to help if

    No improvement by 8 weeks with repleted levels: investigate iron/ferritin, TSH, sleep quality, and mood.
