    Efficacy of heavy eccentric calf training for treating mid-portion Achilles tendinopathy: a systematic review and meta-analysis

    Abstract

    Systematic review and meta-analysis evaluating the efficacy of the heavy eccentric calf training (Alfredson) protocol in reducing pain and improving function in patients with mid-portion Achilles tendinopathy.

    Methodology

    Search of databases for RCTs implementing heavy eccentric calf training protocols for mid-portion Achilles tendinopathy; outcomes pooled for pain (VISA-A) and patient-reported function.

    Key Findings

    Heavy eccentric calf training led to clinically meaningful improvements in pain and function; effects were comparable to or better than some other conservative treatments.

    Conclusions

    Heavy eccentric calf training is an effective conservative treatment for mid-portion Achilles tendinopathy and should be considered a first-line intervention.

    Limitations

    Variation in eccentric protocol parameters across trials; moderate risk of bias in some included studies; small number of eligible RCTs.
