Comparison of two diets for the prevention of recurrent stones in idiopathic hypercalciuria

Methodology
Five-year randomised controlled trial comparing a normal-calcium, low-animal-protein, low-salt diet with a low-calcium diet in men with recurrent idiopathic calcium oxalate stones and hypercalciuria; recurrence confirmed radiologically.
Key Findings
In 120 men with recurrent calcium oxalate stones and hypercalciuria followed for 5 years, a diet with normal calcium (30 mmol/day) but restricted animal protein and salt halved stone recurrence versus a traditional low-calcium diet (20% vs 38% recurrence; relative risk 0.49), and reduced urinary oxalate excretion.
Conclusions
A normal-calcium, low animal protein, low salt diet halved recurrent stone risk versus a traditional low-calcium diet over 5 years.
Limitations
Men only, single Italian centre; unblinded dietary intervention with self-reported adherence; findings may not generalise to women, non-hypercalciuric stone formers or other dietary cultures; no supplement arm tested.
Supplements Studied
In men with recurrent calcium oxalate stones and hypercalciuria, restricted intake of animal protein and salt, combined with a normal calcium intake, provides greater protection than the traditional low-calcium diet.
