Heavy-load eccentric calf muscle training for the treatment of chronic Achilles tendinosis

Abstract
Prospective study of 15 recreational athletes with chronic Achilles tendinosis unresponsive to conventional nonsurgical treatment, evaluating the effect of a 12-week heavy-load eccentric calf muscle training program.
Methodology
15 patients performed painful heavy-load eccentric calf raises twice daily for 12 weeks; pain and strength were measured pre/post.
Key Findings
All 15 patients returned to pre-injury activity level with significantly reduced pain and increased calf strength after training, avoiding surgery.
Conclusions
Heavy-load eccentric calf muscle training is an effective, low-cost conservative treatment for chronic Achilles tendinosis, potentially avoiding surgery.

Very small sample size (n=15); no randomization or control group; short-term follow-up; single-center study; potential selection bias.
