Effect of pasteurized Akkermansia muciniphila MucT on insulin sensitivity, body composition, and GLP-1 production in subjects with metabolic syndrome: impact of low baseline gut Akkermansia levels

Methodology
Four-month double-blind, placebo-controlled multicenter trial (Ireland and Germany) of 30 billion cells/day pasteurized A. muciniphila MucT capsules in adults with metabolic syndrome (NCT05114018), with deep metagenomic microbiota analysis.
Key Findings
The primary endpoint of whole-body insulin sensitivity (Matsuda index) did not differ from placebo in the intention-to-treat population. Exploratory analyses showed improved HOMA-based hepatic insulin sensitivity in prediabetic and older subgroups and greater post-OGTT GLP-1 excursion; participants with low baseline Akkermansia gene counts improved insulin sensitivity, GLP-1 excursion and trunk fat.
Conclusions
Pasteurized A. muciniphila MucT may improve metabolic markers mainly in people with low baseline intestinal Akkermansia; the overall trial did not meet its primary endpoint.
Limitations
Primary endpoint null; subgroup benefits are exploratory. The trial was run by The Akkermansia Company, the product manufacturer.
Supplements Studied
